      The Miracle Foundation Palliative Care Center

The goal of The Miracle Foundation Palliative Care Center is to make available a greatly needed resource at a difficult and often confusing time in one's illness. The focus is to provide comprehensive, supportive care for the patient, the caregiver and family members through home-based palliative care visits.

The Miracle Foundation Palliative Care Center is based on a model of care which is gaining ground as the gold standard of supportive critical care. The goal is to help patients live the most productive and best possible quality of life at perhaps the most difficult time of their illness.

In partnership with the Hospice Care Network, The Miracle Foundation Palliative Care Center fulfills a previously unmet need for patients and families on Long Island, at a time of change, crossroads or crisis in one's illness.

Between July 2012 and August 2013, The Miracle Foundation Palliative Care Center admitted 159 patients and provided services in the home, to patients in the hospital and at outpatient treatment facilities.

Expanding on this progressive initiative, a Medical Fellowship in Palliative Care will begin in January of 2014. A focus of this Fellowship will be home-based palliative care visits.
